An Ohio woman underwent a total abdominal hysterectomy, during which her bladder was injured and immediately repaired. The patient was discharged without a Foley catheter. One week later she returned to the ER and was diagnosed with right ureteral vaginal fistula. Three weeks later corrective surgery was performed. A medQuest GYN surgeon reported it was below the standard of care to fail to recognize or repair the injury to the ureter in a timely fashion. A medQuest urologist reported that the negligent surgery led to the need for subsequent surgeries and the patient's chronic urinary problems.
